Every cohort runs the same protocol. Results get measured and reported on your facility’s own data. Not borrowed from someone else’s brochure.

Tracks

Assessment battery and thresholds follow published geriatric exercise research (CDC STEADI, Rikli & Jones Senior Fitness Test). Outcomes are tracked and reported on your facility’s own cohort data.

The measure that matters isn’t how many residents showed up. It’s how many moved out of high fall-risk by Week 12.
The Crucible Care lens

The program

Twelve weeks. Three phases. One outcomes report.

Same protocol every cohort. Quality first, load second, performance third — measured along the way and printed at the end.

Phase 01Weeks 1-4

Foundation

Movement quality + confidence

Residents learn the eight foundational moves and rebuild trust in their own bodies. We test before we load. Coaches catch the residents who need a chair-based variant before week 2.

  • 8 scripted sessions
  • Baseline assessment in week 1
  • Mobility + sit-to-stand emphasis
Phase 02Weeks 5-8

Build

Progressive loading

Resistance climbs week by week. Bands become free weights. Reps become tempo work. The midpoint assessment in week 6 tells the coach who's ready to push and who needs another foundation block.

  • 8 scripted sessions
  • Midpoint assessment in week 6
  • Lower-body power + push/pull emphasis
Phase 03Weeks 9-12

Perform

Functional performance

Strength translates to walking confidence, single-leg balance, and grip carry. The things that matter when a resident reaches for a railing. The final assessment in week 12 produces the outcomes report a regional VP can act on.

  • 8 scripted sessions
  • Final assessment in week 12
  • Balance + grip-carry emphasis

Crucible

A vessel that withstands extreme heat. The place where iron is forged into steel.

The name is the program. Twelve weeks under structured load is what helps deconditioned residents move with more strength, control, and confidence during daily activities.

FAQ

What your board will ask.

Does my staff need fitness certifications?

No. Every session is scripted: warm-up, circuits, balance work, coaching cues. Activity directors and wellness coordinators run it after a two-hour onboarding.

How is this different from what our PT department already does?

PT is medically driven, episodic, and insurance-billed. Crucible Care is a structured wellness program. It runs between therapy episodes and helps facilities document strength, balance, participation, and mobility trends before residents need higher levels of support. Most facilities find the two work alongside each other, not instead of each other.

What if a resident falls during the program?

Every participant clears a screening before Week 1. High fall-risk residents go into the Chair-Based track, which is entirely seated. Every session includes written stop criteria and an escalation protocol to nursing. Release templates and incident documentation forms are included.

I need my clinical director or DON to sign off. Can I send them something?

Yes. We have a one-page clinical summary covering assessment methodology, safety framework, medical screening process, and the safety documentation set. Email care@crucible.fit and we'll send it within one business day. Or visit the Safety page for the full framework.

Can I see an actual outcomes report before buying?

Yes — the sample outcomes report is live on this site (link in the hero). Same structure your facility's report will use, rendered in your browser. No download, no email request, no wait.

How does this affect our survey readiness?

The outcomes report documents structured wellness programming, resident participation, standardized assessment results, and fall-risk category shifts. This is the kind of systematic data surveyors ask about under quality of life and quality of care. We can walk you through the document and let you evaluate it yourself.

Pricing

Priced against a fall. Not your activities budget.

Most facilities ask this first, so here’s a straight answer: the program is priced against the cost of a single resident fall — not against your activities budget. There are two ways to run it: you run it — your wellness staff lead the program on our platform and we train and support them (the most affordable option, and most facilities’ long-term choice); or we run it — our coach is on-site each week. Exact pricing depends on your community’s size and which option fits, so we quote it on a quick call after you’ve seen the program work. The best place to start is a free demo class.

Outcomes guarantee: If your cohort completes 12 weeks and we can’t generate an outcomes report from your data, we refund the program fee in full.
